School support delivery tracker
Keep agreed support, what happened and your next steps together. Use this with an IEP, EHCP, IDP, IPP, statement or other school support arrangement.
The same plan name can have different legal status across regions. Separate confirmed facts from information you still need to check. Use the regional support-delivery guide for local next steps.
Your entries stay in this page's memory. This tool does not send entries to us or save them automatically. Export before closing or refreshing the page; entries cannot be restored here from an exported file. Use a learner alias rather than a full name, school name or medical details. Keep any downloaded records somewhere private.
Start with one specific support requirement. Quote the plan wording where possible. Suggested follow-up dates are your planning dates, not calculated legal deadlines.
See a fictional example
Agreed support: “A visual checklist for multi-step classroom tasks.” Observation: “Learner says the checklist was unavailable on Tuesday; delivery not yet confirmed.” Evidence: “Email sent to case manager on Wednesday.” Next step: “Ask staff to confirm availability and agree a review date.” This example describes a query, not proof of a legal breach.
What goes into the export?
The CSV contains your education-system selection, alias and each record. Spreadsheet formula-like text is prefixed with an apostrophe for safer opening. Blank records are omitted. The printed summary uses the same entries. This tool does not calculate entitlements, missed-service awards or appeal deadlines.
What to do with your record
- Check the current plan and note its date or version.
- Ask the school to clarify gaps you have not confirmed.
- Record the response, the agreed action and who will follow up.
- Use your local guidance if concerns remain unresolved.
