Transition planning with a Statement of SEN in Northern Ireland

Start early

Do not wait until the final term before a change. Ask when the relevant annual review will address placement and transition, what information the receiving setting needs, and who is responsible for coordinating the move.

What to cover

• the pupil’s views and concerns
• support and strategies that currently work
• communication, sensory, medical or accessibility needs
• specialist provision in the statement
• orientation visits or phased transition
• transport and practical arrangements
• transfer of records and professional advice

Post-primary placement

Current EA admissions guidance states that children who already have a Statement of SEN do not use the ordinary admissions route for placement at transfer; placement is handled through the SEN annual-review process.

Age 14 and beyond

EA’s Statutory Assessment and Review Service includes a Transitions Service 14+. Use reviews in the teenage years to discuss education and training pathways, independence, participation and any agencies that should be involved in planning for adulthood.

Record agreed actions

After each transition meeting, ask for the actions, responsible person and expected date in writing. If the statement itself needs to change because needs, provision or placement are changing, ask the EA how and when amendments will be considered.
