EHCP evidence checklist: what to include with your request
Aim to show the child’s needs, the support already tried, what happened, and why an EHC needs assessment may be necessary.

Useful evidence
- School or nursery reports and SEN Support records.
- Intervention and progress records.
- Relevant professional reports you already have.
- A concise parent/carer account.
- The child or young person’s views where appropriate.
A simple structure
Organise evidence as: need → support tried → result → remaining difficulty → why further assessment may be needed.
